R.C. Hospice Care is a family-owned agency that has dedicated itself to the families of its’ community. This organization is a legacy of the owner’s father; founded on care and compassion towards the human life, near the end of our journeys. It is important to approach one’s final journey with the utmost respect, care, and dignity.

Hospice is when an individual is diagnosed with a terminal illness and has no cure. No cure does not mean no care. Hospice provides medication and symptom management, medical equipment and complete care planning tethered to your terminal illness. Our Services include a Hospice Physician, Case Manager Registered Nurse, Home Health Aides for all hygiene and personal care, Medical Social Worker for all resource and comfort needs and a Chaplain for all your spiritual needs. Our organization also has an exceptional volunteer program.

What should you know before electing your hospice benefit?

Everyone who has insurance may be eligible for the hospice benefit. The hospice benefit is a covered service that includes the following: clinical care team, medications related to your diagnosis, counselors, bereavement and volunteer coordinators. Under our program you will receive management and supplies which includes:

MEDICATION

  • Medication Management

  • Orders filled through Hospice program

  • Medication delivered to your door

  • Most Hospice Benefits do not have copay for medications

DURABLE MEDICAL EQUIPMENT

  • Hospital Beds – full electric

  • Alternating pressure pump mattress

  • Wheelchair all sizes – including recliners

  • Commodes

  • All equipment that will be needed for your comfort

HYGIENE SUPPLIES

  • Incontinence Supplies

  • Catheterization

  • Wound care supplies

  • All supplies that your Aide will need to bring you comfort.


The RC Hospice plan of care coordination

1. CONSULTATION

Give us a call—we are always here to do a courtesy nursing consultation in the comfort of your residence. We relay all information back to your primary care physician to better understand if you qualify for hospice care services.

2. INTAKE REFERRAL

Upon receiving the referral order from your Physician, our intake coordinator will contact you to arrange the delivery of your medical equipment and admission time.

3. ADMISSION

Your Case Manager will then meet you wherever you reside, such as assisted living facilities, residential care facilities and skilled nursing facilities to admit you to the program.

4. PERSONAL PLAN OF CARE

Together we will tailor your Plan of Care with our services and team to meet your needs.

5.MEDICATION MANAGEMENT

Your medications will be ordered, obtained, delivered and managed by the hospice care team.

6.SYMPTOM MANAGEMENT

All symptoms including any sort of pain, discomfort and wound care will be managed by your personal clinical team. We believe in continuity of care.
